CareOne Long Term Acute Care Hospital (LTACH) at Trinitas Regional Medical Center offers a specialized level of high-touch care for patients that are transitioning out of ICU.

Located on the 7th floor of Trinitas Regional Medical Center, this LTACH is a uniquely designed hospital setting for patients with complex medical needs requiring a longer recovery time.

Patients who are seriously ill and receive care in an LTACH can significantly reduce the likelihood of being readmitted to the (ICU) while also improving treatment outcomes. CareOne’s skilled team of board-certified physicians, experienced nurses, physical therapists, registered dietitians, and social services professionals work closely together to deliver comprehensive and collaborative treatment.

    Our physicians have experience with complex wound management and treatments. Our specially trained and certified wound care nurses provide our patients with the highest level of care for serious and non-healing wounds.

    Our complex cardiac care management includes expertise in telemetry, arrhythmia management, external pacing, heart failure treatment and monitoring, hemodynamic monitoring, cardioversion, TCP and cardiac monitoring.
