Diabetes Management at CareOne
CareOne develops highly personalized diabetes care plans for each patient’s specific needs, including the cultural, religious and financial aspects of dietary management that may impact food choice or consumption. From there, interdisciplinary teams work closely together to improve each patient’s functioning and prevent potential re-hospitalizations through careful glycemic management, early identification of symptoms, and prompt treatment protocols. These teams of specialty physicians include endocrinologists, diabetic nurse educators, registered dietitians, nurse program directors and medical directors.
By developing comprehensive clinical profiles for our patients, we can manage the complications associated with diabetes and help improve overall quality of life.
We offer various modalities of care, including:
- Point of care testing for blood glucose

Additionally, our care program offers various therapeutic services with treatment up to seven days per week. This includes skilled therapy assessments, functional mobility training, medication system training, and self-care and energy conservation strategies.
Also, CareOne provides a range of community support resources from the American Diabetes Association as well as partner hospital-based programs and support groups.
