What is Hospice Care?

Our Approach to Hospice Care

We help patients and their families navigate the challenges of a life-limiting condition through personalized end-of-life care. In partnership with Ascend Health, we coordinate hospice care services with professionals who specialize in supporting these unique emotional, physical and spiritual needs.

Inpatient care is available in our skilled nursing facilities, where residents in hospice have their own rooms for privacy. Through Ascend, we can also help you transition to hospice at home.
